TriCore Reference Laboratories (TriCore) — http://www.tricore.org/ — is seeking to assist other organizations in the way medicine is delivered by creating laboratory-driven population health management and targeted intervention tools. Their efforts focus providing clients solutions within the healthcare paradigm shift from fee-for-service to value driven healthcare.

Timely identification of patients in need of care coordination requires real-time data indicating the reliance of latent claims data ineffective. Laboratory results account for 3% of annual healthcare spending yet they influence up to 70% of disease diagnoses and management decisions. Therefore, laboratory data may increase the effectiveness of care coordination by identifying patients at risk of comorbidities associated with high cost high occurring health conditions such as pregnancy, hepatitis C, and diabetes. Tricore relies on longitudinal patient history to determine risk for complications to support timely decisions regarding continuity of care that could lead to improved outcomes and decreased costs.

In the state of New Mexico, TriCore Reference Laboratories provides laboratory services for approximately 60-70% of the New Mexico population with a broad geographic foot print containing over 21 clinic sites and 28 draw sites that perform 9 million tests annually. With almost 20 years of longitudinal, historical laboratory results and support from more than 45 pathologists, TriCore is in a unique position to provide actionable data that can support MCOs in their efforts to supply effective care coordination.

Charles R. Rath is the President & CEO of Resilient Solutions 21 (RS21), a global consultancy created to help communities, cities and businesses flourish in today’s world. RS21 specializes in creating insightful, web enabled analytics that inform some of the world’s most difficult national security challenges. The company’s current portfolio of work includes innovative risk and resilience projects with Rockefeller’s 100 Resilient Cities Challenge, the US Department of State, US Department of Homeland Security, and cities around the globe. Before starting RS21, Rath led the Resilient Cities Program at Sandia National Laboratories (SNL), a program developed to provide technical assistance to help cities overcome the shocks and stresses of the 21st century. Prior to joining SNL, Rath served as Deputy Director in the Department of Homeland Security (DHS), where he built analytic capabilities and delivered risk analysis to decision-makers at the federal and local levels

Rick VanNess will present TriCore’s new analytic tools that emphasize on creating actionable data to help identify patients in need of care. Rick is a Product Manager within TriCore’s Diagnostic Optimization team and is tasked with differentiating TriCore to help its clients achieve better outcomes by closing care gaps and increasing patient experience. He earned his Bachelor of Science in Environmental Health at New Mexico State University and his Master of Science in Pharmaceutical Sciences at the University of New Mexico.
